The process

From referral to admission

  1. 01

    Send the referral

Call the admissions office or fax clinical records. We accept referrals from hospital case managers, discharge planners, physicians, insurers, families, and county care teams.

  2. 02

    Clinical review

A clinical liaison reviews the chart and, when helpful, travels to your facility to assess the patient in person — often the same day.

  3. 03

    Benefits verification

We verify insurance coverage and obtain authorization, then review expectations for length of stay and level of care with the family.

  4. 04

    Admission & transport

Our critical care EMS team can transport the patient. The physician, nursing, therapy, and case management teams are briefed before arrival.

Online referral

Send a referral inquiry

Complete the form below and it goes straight to our admissions inbox at info@lakeviewsystem.com. A liaison follows up by phone, usually the same business day.

Please do not include full names, medical record numbers, or other protected health information in this form. An admissions liaison will contact you on a secure line to collect clinical records.

For referring providers

What to send with a referral

Send what you have — we will follow up for anything missing rather than delay the review.

  • History and physical, plus recent progress notes
  • Current medication administration record
  • Recent labs, imaging, and culture results
  • Ventilator, dialysis, or wound care orders as applicable
  • Insurance information and demographics
  • Therapy evaluations, if completed

For families

You are allowed to ask hard questions.

Families come to us in the middle of the hardest weeks of their lives. Ask us about length of stay, who will manage the ventilator, how therapy is scheduled, what discharge could look like, and what insurance will cover. If Lakeview is not the right level of care, we will say so and help you understand the alternatives.

You are welcome to tour the hospital before deciding. Call the admissions office to schedule a visit.
