When to refer for long-term acute care
What will the next hospital need to do on the first shift? If your patient is stable for transfer and still needs daily hospital treatment, Lakeview Admissions can review the case.
Describe the airway and the latest trial
Include the airway, current settings and recent changes. Tell the liaison how the patient responded when support was last reduced and how often suctioning is needed.
Other treatment that must continue
List the IV therapies, wound treatments and dialysis orders that will continue. Flag the treatment that requires the closest monitoring.
Records for the first review
Send the history and physical, recent notes, medication record, relevant results and current orders. Include available therapy evaluations. Our liaison can identify any gaps.
