Community based residential facilities
A home, with the clinical backing of a hospital
Some people are ready to leave the hospital but not ready to live without support. Our CBRF homes provide that middle ground — a family-style household where residents are known, not managed.

Who we serve
Adults living with cognitive, neurological, and behavioral challenges
Many of our residents arrive after a brain injury, a long hospitalization, or years of placements that did not fit. What makes our homes different is the family-style atmosphere — between staff, residents, and involvement in the surrounding community, residents feel like they are at home.
We work closely with families and care teams to make sure each resident has the strongest possible support system, and we adjust supports as independence grows.
Daily support
What life looks like
24-hour supervision
Trained staff on site at all times, with nursing oversight and medication administration.
Neurobehavioral supports
Individualized behavior support plans developed with our hospital's neurobehavioral clinicians.
Daily living skills
Cooking, budgeting, hygiene, and household routines practiced in a real home, not a classroom.
Community integration
Work, volunteering, recreation, faith communities, and family visits are part of the plan, not exceptions.
Therapy & clinic access
Residents can access Lakeview's outpatient therapy and behavioral health clinic as needs change.
Family partnership
We work closely with families, guardians, and county care teams so support systems stay strong.
