Choosing between Lakeview’s hospital services
For a Lakeview referral, describe what the patient needs medically and what they want to do again. Those two details help our liaison review long-term acute care and inpatient rehabilitation options.
When hospital treatment drives the plan
Tell us what a physician, nurse or respiratory therapist must assess or adjust each day. Include ongoing ventilation, IV treatment, dialysis and complex wounds where relevant.
Inpatient rehabilitation
Describe what the patient did independently before illness, what assistance they need today and how they tolerate therapy. Include medical needs between sessions.
When the fit is unclear
When a patient has both respiratory needs and therapy goals, give the liaison both parts of the picture. Ask which service should assess the referral first.
From referral review to a safe handoff
Clinical acceptance, coverage and transport are separate decisions. Confirm the status of each before telling the patient or family that a move is scheduled.
During review
Give the liaison a direct clinical callback number. Send an update if a new result, procedure or change in support affects the proposed transfer.
Before departure
Confirm the destination, acceptance and transport support. Review last doses, airway and feeding needs, and the first treatment due after arrival.
At handoff
Name the clinician following each outstanding result. Confirm where the family should go and how they can reach the receiving unit.
