Hospital care and referrals

When discharge still requires hospital-level care

A useful referral describes the care the patient needs between now and tomorrow morning. Tell Lakeview Admissions which treatments must continue and what help the patient needs between them.

Hospital care after the first hospital stay

When extended hospital care is being considered

A long-term acute care hospital can review patients who are stable enough to transfer but still have hospital-level needs. The sending team and the receiving liaison should discuss four separate issues:

  • Which treatment needs daily physician decisions, nursing or respiratory assessment?
  • What is the patient’s current care setting and what resources will the receiving team need?
  • Which clinical, coverage or transport decision must be made before transfer?
  • Who will communicate the plan to staff and the patient or family?

Clinical Excellence in LTACHs

  • Specialized Care Teams: Led by sub-specialized physicians, teams encompass nurses, dietitians, rehab specialists, and more, ensuring holistic care.
  • Respiratory Care: 24/7 respiratory therapist coverage, focusing on liberating those on extended mechanical ventilation.

Hospital capabilities to confirm before transfer

  • Lakeview provides hospital treatment, respiratory therapy, pharmacy support, rehabilitation and on-site dialysis. For telemetry, specialized testing, imaging or equipment, confirm the current patient-specific requirement with the liaison.
  • Communicate any infection, isolation requirement or need for special room arrangements before transfer. Confirm that the receiving team can provide the required precautions and plan of care.
