Based on a person’s individual needs, the care and services are designed to be highly functional for that individual. Goals are more effectively and efficiently achieved, because they are practiced in familiar environments, from home to community to workplace.

Finding a program that works for the individual…

Functional goals and objectives are the focus for each individual served by Lakeview Rehab at Home. As a person enters the program, their medical status and history, previous evaluations and consults, current medical recommendations and individual goals and resources are reviewed through a

personalized assessment. The Clinical Coordinator develops program recommendations with the individual and family, as appropriate. When initial professional team representatives are identified, additional assessments to identify a program of treatment and services are conducted. As services are delivered, new resources may be identified and brought into the person’s circle of care. Community resources, such as church, health club, social organizations, as well as vocational and educational resources are included where appropriate.

Communication, quality services, and person-centered program are key…

Each person’s clinical coordinator and all team members

communicate directly with the person receiving services, as well as participating in team conferences and sharing written results of assessments and therapy. The clinical coordinator also communicates with sponsoring agencies and leads a team focus on quality of service and meaningful outcomes.

The individualized program can serve children, adults, and seniors. Services are offered to persons with:

  • Brain injury
  • Stroke and other neurological impairments
  • Complex care issues
  • Alzheimer’s disease (including early onset) and degenerative disorders
  • Atypical developmental issues
  • Multiple trauma recovery
  • Spinal cord injury
  • Autism, spectrum disorders
  • Dual diagnosis (as well as other complex diagnoses in children, adolescents, and adults)

A variety of payment plans, including private pay, commercial insurance, workers compensation insurance, Medicaid and Medicaid waiver program funds, are accepted.
